Personnellement je fais des formulaires via VBA pour les utiliser dans les Macros mais je sais qu'il y a une autre pist.
Voici le help sur la création
Create a new custom form
Any custom forms you create in your active project file are stored only in that file. They are not automatically available in any other project files that you have created previously or that you may create later. However, you can use the Organizer to copy a custom form to your global file if you want it to be available for all your project files.
On the Tools menu, point to Customize, and then click Forms.
Click Task or Resource, and then click New.
In the Name box, type the name of the new custom form, and then click OK. If you want, you can assign a shortcut key for displaying the form in the Key box.
In the Custom Form Editor, click Text, Group Box, Button, or Fields on the Item menu. If you click Button on the Item menu, click OK or Cancel, and then click OK.
If you click Fields on the Item menu, you must select the field, specify its size and position, and then click OK. On the File menu, click Save.
On the File menu, click Exit. Tips
You can edit any component you add to a form by double-clicking the component to display the Item Information dialog box. This dialog box enables you to specify the component's position using x and y coordinates, width and height, and text.
You can move any component in a form by dragging it within the form.
You can size any component by dragging the top, bottom, or side border. If you drag a border corner, the component is sized proportionally.

Ces formulaires servent à remplir de l'information sur les projets, dans des champs qu'on choisira selon les besoins. Des exemples de ces formulaires sont Informations sur la tâche et Avancement des tâches...
Ces formulaires peuvent être pratiques pour éviter d'utiliser plusieurs formulaires, ou encore pour concentrer les formulaires selon une activité de planification ou de suivi spécifique.
À noter que ces formulaires sont des objets et qu'ils sont donc transférables par l'Organiseur. À noter aussi qu'en version 2000, certains champs ont été oubliés (WBS, entre autres).
